Test Code 01C P2Y12 Test

Important Note

Patients who have been treated with Aggrastat or Integrilin within two days or Reopro within two weeks should not be tested.

Patients with inherited platelet disorders such as Von Willebrand Factor Deficiency, Glanzmann Thrombasthenia and Bernard-Soulier Syndrome have not been studied with the Verifynow P2Y12 assay. Verifynow P2Y12 assay is not intended for use with these types of platelet disorders.

Test reliability not studied for patients with:

  • Hematocrit: >52%
  • Platelet Count <100,000 or >502,000/mcL
  • Fibrinogen >599 mg/dL
  • Cholesterol > 316 mg/dL
  • Triglyceride > 824 mg/dL

Test Method

Platelet Aggregation Inhibition, Verifynow; Accumetrics

Specimen Requirement

Collect two Greiner 2.0 mL partial fill 3.2% sodium citrate tubes using a 21 gauge needle. 2 mL of whole blood is required for testing.

Additional Specimen Collection Information

Collect whole blood in a Greiner 2.0 mL partial fill 3.2% sodium citrate tube using a 21 gauge needle. No other sample types are acceptable. Samples should be collected between 2 and 30 hours after ingestion of aspirin.

Venipuncture collection instructions:

  1. Collect two Greiner 2.0 mL partial fill 3.2% sodium citrate tubes using a 21 gauge needle. The tubes must be filled to its intended whole blood capacity (indicated by a black triangle). Indicate on the label which tube was drawn first and which was drawn second. The first tube is not used for the P2Y12 Test.
  2. Do not draw a sample for any platelet function assay after a tube that contains EDTA purple-top. Always obtain blood in the EDTA tube after drawing the tube for the platelet function assay.
  3. Immediately mix the citrated tube containing whole blood 5 times by very gentle inversion.
  4. Samples must be kept at room temperature.  Do not refrigerate, freeze or centrifuge the sample.
  5. Blood must set for a minimum of 10 minutes after collection before assay, but no longer than 4 hours.

Indwelling catheter collection instructions:

  1. Discard the first 5 mL from an indwelling catheter to clear the line.
  2. Ensure the catheter is free of clots.
  3. Immediately transfer blood to a Greiner 2.0 ml partial fill 3.2% sodium citrate tube.
  4. The tube must be filled to its intended whole blood capacity (indicated by a black triangle). Do not under fill.
  5. Do not draw a sample for any platelet function assay after a tube that contains EDTA purple-top. Always obtain blood in the EDTA tube after drawing the tube for the platelet function assay.
  6. Immediately mix the citrated tube containing whole blood 5 times by very gentle inversion.
  7. Samples must be kept at room temperature.  Do not refrigerate, freeze or centrifuge the sample to be used for the verifynow assay.
  8. Blood must set a minimun of 10 minutes after collection before assaying.

Storage conditions:

Specimens can be stored for up to 4 hours at room temperature. Do not refrigerate or freeze samples.

Performance

Testing is performed Monday through Sunday.

Typically completed within 1 hour after the specimen arrives at the testing site.

Reference Values

180 - 376 PRU

Values < 180 PRU are specific evidence of a P2Y12 inhibitor effect.
